Please be prepared to answer questions regarding your medical condition and reason for medical leave. This includes dates, names of treating healthcare providers, medications, surgeries, referrals, diagnoses, and any other relevant information. You will receive your completed Attending Physician Statement via e-mail (PDF format) within 24 hours of your appointment start time.

Your form (Attending Physician Statement or APS form) needs to be submitted to your disability company in order for your claim to be reviewed and approved. There is often an employee portion that you will need to fill out and submit alongside your completed disability form. Please note that Phzio is not responsible for submitting your paperwork. In rare cases, the form needs to be faxed directly from the signing provider. If this is the case, please advise us during your appointment.

Your application will be reviewed by the disability management company or major health carriers that work with your employer. Please direct all questions about your application to the person managing your disability case.
Simple errors can be corrected and your APS form can be reissued to you at no cost. You will be able to flag errors and your signing provider will fix the form. If you need a reassessment or a leave extension, there will be a fee and you will need to schedule a new appointment.
While we do have availability on weekends, these time slots fill up fast. Please do not leave the appointment until the last minute.

Phzio does not replace the treatment you have been receiving for your medical condition. Please note that while our team can help you fill out your APS form, they will not prescribe any medications or order any tests for you and will not take over the role of treating provider.
