You will have a virtual/online assessment with a Nurse Practitioner. They will complete your form and you will receive it via email (PDF format) within 24 hours of your appointment.

Your forms (Attending Physician Statement APS Form) will be ready to submit to your disability company. You will typically also need to submit your employee statement and consent forms as well.

Your application will be reviewed by the disability management company that works with your employer. They will be able to tell you the status of your application.
Simple errors can be corrected and your form can be reissued to you at no cost. If you need a reassessment or a leave extension, there will be a fee.
